I acknowledge that I am required to fulfill 90 days of rental which includes a full final month (no prorated amounts). If I move out prior to this day, my card will automatically be charged the remainder of the required rent. (Please initial)

I acknowledge that I am required to submit a 15-day move out notice prior to the end of the month when moving out. The last month is not prorated. If notice is not given in time, another full month will be charged. (Please initial)
